Description of the Problem
Address of Violation*
  Reporting the correct address is very important. Example: 2530 Middle St. NW. If a vacant lot, provide the block number or the direction from a known address. Example: "Vacant lot on the west side of 2530 Middle St. NW" or "Vacant lot on the northeast corner of Middle St. and Main Ave. S."
NOT: "On the corner of Middle and Main."
Description of Violation*
  As an example: "junk car in yard," or "dilapidated garage," or "broken windows," or "tall grass," or "junk in yard." NOT: "Looks bad," or "needs to be condemned."
