Requests are taken year-around. Absentee ballots are available 46 days prior to each election.

Minnesota Military and Overseas Voter Service
- Active-duty uniformed service members and their families
- US citizens living abroad permanently or temporarily
- Please allow sufficient time for ballot delivery and return based on your location
*This application need not be on an official or standard form. It will be accepted if it contains all of the following information, including the oath written out. Please allow at least seven days to complete this process. Copy content from the Absentee Ballot Application, or use this list:
1. Reason for need an absentee ballot (choose one): absence from precinct, illness or disability, religious discipline or observance of religious holiday, service as an election judge in another precinct, or eligible emergency declared by the governor or quarantine declared by federal or state government
2. Absentee ballots requested for the following election(s): both the primary and general elections, primary only, general election only, special primary – include date, special general election – include date
3. Name – last, first and middle names, suffix (if any); Phone number and email address are optional, but very helpful if we must contact you with a questions about your application
4. Date of birth
5. Your MN Driver’s License or State ID number; the last four digits of your social security number; or state that you have a MN DL or MN ID or SSN #
6. Address where you live
7. Address to which you wish ballot(s) sent – if different from #6
8. Print out this entire oath, date and sign the application:
I certify that I:
- Am completing this application on my own behalf
- Will be at least 18 years old on election day
- Am a citizen of the United States
- Will have resided in Minnesota for 20 days immediately preceding election day
- Maintain residence address at the address given on this application form
- Am not under court-ordered guardianship in which the court order has revoked my right to vote
- Have not been found by a court to be legally incompetent to vote
- Have the right to vote because, if I was convicted of a felony, my felony sentence has expired (been completed) or I have been discharged from my sentence; and
- Have read and understand this statement: The above information is accurate, and I sign this application form under penalty of perjury, a felony punishable by not more than 5 years imprisonment, a fine of not more than $10,000, or both.
