When you are conducting a voter registration drive, understand the eligibility requirements for voter registration listed in the oath at the bottom of the application. Read the information and notices on both the front and back of the application, including the Confidentiality Notice .
All shaded areas of the MN Voter Registration Application must be completed by the voter.
Do not fill out any portion of the application for the voter unless assistance is required.
Voters who are already registered should re-register if they have
- Moved to a different address (including from one apartment to another apartment within the same building)
- Changed their name
- Have not voted for four consecutive years
Please use an ink pen and print clearly.
Information required by the Federal Help America Vote Act (HAVA)
In Lines 1 & 2, applicants must answer the questions at the top of the application
- "Are you a U.S. citizen?"
- "Will you be at least 18 years old on or before the next election?"
- Note: Voters must be 18 or older on the day of the day they vote. If an individual is 17 on the day of the Primary, and 18 on or before the General Election day, the individual cannot vote in the Primary and must wait until the General Election to vote.
In Line 7, all registrants must provide their Minnesota Driver’s License or Minnesota ID number if they have one
- Registrants who do not have a MN Driver’s License Number or MN ID number must provide the last four digits of their Social Security number
- Registrants who do not have a MN Driver’s License, MN ID or a Social Security number must check the box declaring that they have neither item

Felony Status
Registrants with a question on the status of a prior felony conviction should contact their probation officer to gain clarification. Applicants with questions about their status should not complete a voter registration application until they know that their civil rights have been restored.
Returning Completed Applications
Minnesota law (M.S. 201.061 subd.1) requires completed Voter Registration Applications be submitted to election officials within 10 days after the voter dated the application.
Pre-registration deadlines are at 5:00 PM, 20 days prior to each election. Check with Election Officials for the pre-registration deadlines for special elections.

Mail in Applications: If you mail in the applications collected at a voter drive, please include a copy of your business card or contact information with the applications. Applicants who submit their applications by mail, either through the United States Post Office or a commercial delivery service, may be required to show identification at the polls the first time they vote in a federal election.
