Workers’ Compensation Medical Information

The City of Minneapolis has enrolled with Alpha, a Minnesota Preferred Provider Network, to provide all necessary medical management and billing for worker’s compensation injuries. On this page you will find information for employees, supervisors and physicians.

For Employees

Upon reporting a work-related injury, employees will be assigned to one of two Preferred Provider Clinics - HealthPartners Riverside Clinic or Occupational Medicine Consultants – chosen by your department for treatment. Listed below are brochures that outline employees need to know about getting care at these clinics.

For Supervisors

Upon receiving an employee report of injury, supervisors are required to fill out a Supervisor Report of Injury. Listed below is a checklist for work injuries for the clinic chosen by your department.

For Physicians

Upon reasonable treatment of work-related injury, physicians are required to provide a “Report of Work Ability” form to the employee. This form is required for the employee to return to work at the City of Minneapolis.
