- You can obtain medical and immunization records at the clinic where you received services.
- If you came to the City of Minneapolis Health Department for services before 1996, you can call (612) 673-2301.

If you don’t have health insurance you can call the Neighborhood Health Care Network at (651) 489-2273. This network is an alliance of community clinics in the Twin Cities and surrounding areas for low income and uninsured people. They can help you find the nearest clinic to your home.
