What we do

The Healthy Living Initiative (HLI) is rooted in promoting health equity through trust, hope, and healing around healthy eating, active living, and wellbeing. Our efforts center community-defined goals and engage partners in neighborhoods, schools, worksites, and clinics to break down barriers to health and build community-driven solutions that make changes to policies, systems, and environments. 

This initiative receives funding from the MN Statewide Health Improvement Program (SHIP) to work closely with communities in Minneapolis to reduce health disparities. The HLI also works directly to support individuals with diabetes and hypertension through funding from Merck and the Minnesota Department of Health’s 1817 program.

  • HLI is driven by values of health equity and community engagement. Resources are focused on the communities whose health is most impacted by systemic oppression, racism, and marginalization.
  • This health equity work is done with community partners to create place-based opportunities for everyone to eat healthy, be physically active, live tobacco free, and access high quality health care.
