Will you get a $25 check in the mail?

The Rewards for Health Care Shopping Program will soon mail $25 checks to employees who chose a lower-cost health care provider for an imaging service between April 1 and June 30. Eligible employees should receive checks by the end of the month.

It pays to save

If you are an employee on the City’s Medica plan and your doctor recommends that you have an imaging service (a CT scan, a mammogram or an MRI), visit medicahcrewards.com/cityofmpls to compare costs for providers of that service. On the site, the lowest-cost providers are highlighted in green, medium-cost providers are highlighted in yellow and the highest-cost providers are highlighted in red. Any time you receive your imaging test from one of the green lowest-cost providers in the online shopping guide, you will receive a $25 check mailed to your home.

What you need to know to use the website

To use the Rewards for Health Care Shopping Program website, you’ll need to have:

To receive the best benefits for your imaging service, it must be ordered by a doctor in your health plan’s network. That means if you are enrolled in the Medica Elect or Medica Essential plan, the doctor who orders your test must be in your care system.

If you are interested in receiving your imaging test from a lower-cost provider listed in the online shopping guide, let your clinic know so they can help you schedule the appointment. Generally, you can work with your clinic to schedule services with an in-network provider of your choice. If you are enrolled in the Medica Elect or Medica Essential plan, check the shopping guide for providers affiliated with your care system.


All employees and their dependents 18 years and older who are enrolled in one of the Medica plans offered by the City of Minneapolis are eligible to receive this reward. The reward is available for these imaging services. Services received as part of a visit to the emergency department or an inpatient stay are not eligible.

Opt out

If for any reason you don’t want a check for $25 for using one of the lowest-cost providers in the online shopping guide, you may opt out of participating in the Rewards for Health Care Shopping Program by contacting Medica Customer Service at 952-945-8000 or 1-800-952-3455 (TTY users: 1-800-855-2880). When requesting to be removed from the program, please provide your first and last name, address and date of birth.
