Reporting work injuries and other incidents goes online Jan. 1

Starting Jan. 1, the City will switch to an online form to report work-related injuries. The new online form will also create a process to report near misses, internal air quality issues and ergonomics issues.

Moving the form online will:

The online form will be available outside the City’s firewall at a public link, so you will not need remote access software to submit it. You’ll also be able to find the link on CityTalk under work tools, forms and the Risk and Claims Management page.
