Give yourself credit: Self-report your health actions to earn points

Earn points in the employee wellness program by self-reporting these health actions by Aug. 31.

Flu shot: Confirm that you were vaccinated for the current flu season (September 2015–May 2016) to earn 25 points. To self-report your flu vaccination:

Preventive cancer screenings: Confirm that you are up-to-date for breast, cervical or colorectal cancer screening exams that are appropriate for your gender and age to earn 25 points. To view the screenings for which you are eligible and self-report those you completed:

Health and wellness class: Confirm that you attended a worksite health and wellness class for 25 points. You can attend and report up to two classes for a total of 50 points. To self-report a health and wellness class:

Track: Log daily activities in categories such as exercise, healthy eating and life balance to earn a point per day with a maximum 200 points per year. Your daily wellness meter needs to reach 300 before you’ll get a point for that day. You can manually enter your activities or sync your physical activities from a variety of mobile apps and personal devices such as a Fitbit, Jawbone, FitBug, Garmin, MapMyFitness, Runkeeper, Moves or Withings. You can enter data up to 30 days retroactively.

To ensure you meet 300 points by the Aug. 31 deadline, log into today to check your point total and complete your self-reporting.

When you earn 300 points in the employee wellness program by Aug. 31, 2016, you’ll qualify for the discounted wellness rate on your 2016 health insurance premiums. (In 2015, savings on the insurance premiums are about $400 a year for a single person and $1,200 a year for a family.)
