Did you get $25 in the mail?
If you had a CT scan, MRI or mammogram from a low-cost imaging provider between July 1 and Sept. 30, make sure you received a $25 check in the mail in mid-November.
If your doctor recommends you or a dependent get a CT scan, MRI or mammogram, you can earn a $25 check by selecting a service provider that Medica has identified as a low-cost provider. This program, called Rewards for Health Care Shopping, rewards employees for making cost-effective choices about their health care.
Medica, the City’s health insurance provider, offers a list of these low-cost providers at www.medicahcrewards.com/cityofmpls/. You’ll need to know the name of your health plan (Medica Essential, Medica Elect or Medica Choice Passport) and the name of your procedure.
The online shopping guide highlights the lowest-cost providers in green, medium-cost providers in yellow and the highest-cost providers in red. Any time you get your imaging test from one of the lowest-cost providers, you will get a $25 check. Just make sure it’s ordered by a doctor in your network, and make sure to choose a provider in your network. Let your clinic staff know so they can help you schedule the appointment. Generally, you can work with your clinic to schedule services with an in-network provider of your choice.
All employees and their dependents 18 years and older who are enrolled in one of the Medica plans offered by the City of Minneapolis are eligible to participate in this program and earn the $25 reward.
(Services received as part of a visit to the emergency department or an inpatient stay are not eligible.)
