Last chance to earn up to 200 points for a biometric screening

Get 100 points in the employee wellness program and learn about your health status by having a biometric screening that checks your body mass index, blood sugar, cholesterol and blood pressure. You can also get 25 bonus points for each test result within the recommended range (up to 100 additional points).

While worksite screening events are full, you can have your doctor order the tests or use another approved provider.

Screening options
You can have your screening done at a doctor’s office. To do that, go to the screening section in the health and wellness page on and click on “Your Provider.” It will give you instructions and the necessary forms. Make sure your doctor’s office fills out the forms and faxes them to the number on the bottom of the form by Aug. 15 so you receive your points. Please note that while preventive care is covered at 100 percent, if the provider is following up on a condition or using it for diagnostic purposes you might be charged for the appointment.

You may also go through another provider such as LabCorp or Quest Diagnostics for your screening. Both take same day appointments (with at least two hours’ notice), and Quest even has a downtown location in the Medical Arts Building (825 Nicollet Mall, Suite 406). These options are similar to getting an onsite screening; you don’t have to pay for the screening and the results are uploaded automatically into your wellness site. If you schedule a biometric screening at LabCorp and Quest Diagnostics, they will submit your biometric screening form to Medica for you, but you must have your screening by Aug. 4. Read instructions for scheduling a biometric screening. Visit to print the forms needed for these options and to schedule an appointment.

The screening requires measurement of your height and weight and a small blood sample obtained by a simple finger prick. For the most accurate test results, do not eat for at least nine hours before your appointment. You should drink a lot of water before your appointment and take all medications as directed by your physician. You may have black coffee with no cream, milk or sweetener.
