Report a Sidewalk Structural Issue
- This form is for reporting sidewalks that are cracked, broken or uneven.
- This form is not for unshoveled sidewalks. Go to report snow or ice on sidewalk. See snow shoveling rules.
- Questions with an asterisk (*) are required.
- You must provide an email address.
- When you submit the form you will receive an email confirming your request.
- The email will have the case number and an expected completion date for your request.
- Service Level Goal: This issue may take 7 days to resolve.
- Resolution: A site inspection is performed and if necessary, an asphalt patch is ordered. The temporary and permanent repairs are scheduled weather/season permitting.
