Our Healthy Home Specialists do free inspections for families with children under the age of 21.

On this form, we'll ask questions to help us determine: 

• Your child's needs for asthma equipment
• How well your child’s asthma is under control
• Your address and contact information

Your answers:

• Are not legally required
• Are necessary for the program to serve you
• Will be kept secure and used only by asthma program staff
