When you fill out the Adopt-An-Ash Receptacle form, please provide the following information: 

• Your name and contact information
• The address closest to the location requiring an ash receptacle
• The street where you'd like the receptacle placed

By participating in this program, you agree to the requirements on the Adopt-an-Ash Receptacle page.

After you submit your form, the Clean City Coordinator will contact you.
