Use this online tool to apply for property tax exemption by Feb. 1.

Before you begin

Choose the correct form

  • Purely public charities use the CR-IPPC form.
  • Nursing homes and boarding care homes use the CR-NBH form.
  • All other entities use the general tax exemption CR-PTE form.
  • Provide the information outlined or explain why you can't provide it.

Gather property details

  • Property address
  • Parcel identification number
  • Legal description of the property

Include these attachments

  • 501(c)(3) determination letter from the IRS
  • Articles of Incorporation, Charter and Bylaws, including all amendments
  • All active leases in the building or property; include a rent roll and site plan, if available
  • Written description of the organization, its mission, and the recipients of its services
  • Copies of your Federal 990 forms for the past three years.
  • If you don't file 990 forms, provide financial statements, including donations and other revenue and expenses.
  • Resident agreements and facility policies, if applicable

In addition

  • Institutions of purely public charity must provide a completed North Star Factors Survey.
  • Nursing and boarding care homes must provide:
    • Evidence the facility is certified to take part in the medical assistance program (Title 19 of the Social Security Act), or
    • The facility's discharge policy, showing residents are not discharged if they cannot pay.

Charter school applicants have additional requirements

Signature and phone number

  • All applications must be signed and include a daytime contact phone number.
  • One of the following can serve as a signature on the online application.
    • Taxpayer name
    • Name of the taxpayer's authorized agent 
    • Taxpayer Identification Number (TIN)
